Description
Conduct evaluations of patients in both inpatient and outpatient contexts, crafting tailored treatment plans.
Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and functional abilities.
Work side by side with nurses, physicians, case managers, and a variety of rehabilitation specialists to maximize patient outcomes.
Guide patients and their families through therapeutic goals, discharge processes, and essential home exercise routines.
Maintain meticulous documentation of evaluations, interventions, and progress in alignment with institutional and regulatory standards.
Engage in rounds and interdisciplinary meetings to ensure cohesive care across the continuum.
Education
Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution.
Certifications
State licensure as a Physical Therapist.
CPR certification is desirable.
Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not obligatory.
Skills
Prior experience is strongly preferred.
Ability to manage patients with intricate medical and mobility challenges.
Excellent communication and teamwork capabilities.
Familiarity with digital charting and clinical documentation tools.
