Description
Conduct thorough evaluations of patients in inpatient and outpatient environments, creating tailored treatment plans
Deliver physical therapy interventions concentrating on mobility, strength, balance, and overall function
Work closely with nurses, physicians, case managers, and other rehabilitation specialists to maximize patient outcomes
Provide education to patients and their families regarding therapeutic objectives, discharge strategies, and at-home exercise routines
Document assessments, treatment sessions, and patient progress while adhering to facility and regulatory directives
Engage in care team discussions and interdisciplinary meetings to facilitate coordinated patient care
Education
Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution
Certifications
State licensure as a Physical Therapist
CPR certification is preferred
Specialized certification (e.g., wellness, geriatric care) is advantageous but not obligatory
Skills
Prior experience is highly preferred
Capability to treat patients facing complex medical conditions and mobility challenges
Excellent communication and teamwork abilities
Familiarity with digital record-keeping and clinical documentation practices
