Description

Conduct thorough evaluations of patients in both inpatient and outpatient environments, creating tailored treatment plans for each individual

Implement physical therapy interventions that prioritize mobility, strength, balance, and overall functional ability

Collaborate closely with nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes

Provide education to patients and their families regarding therapeutic objectives, discharge plans, and home exercise regimens

Document evaluations, treatments, and patient progress according to facility standards and regulatory mandates

Engage in care team discussions and interdisciplinary meetings to ensure cohesive treatment

Education

Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution

Certifications

State licensure as a Physical Therapist

CPR certification preferred

Specialized certifications (for instance, in neurology or geriatrics) are advantageous but not mandatory

Skills

Prior experience is highly desired

Proficiency in treating patients with intricate medical and mobility challenges

Excellent communication and teamwork skills

Familiarity with using digital charting and clinical documentation systems
