In this role, you will dive into a diverse array of responsibilities, including:

Conducting thorough evaluations of patients in both inpatient and outpatient settings and crafting personalized treatment plans tailored to their specific needs

Delivering physical therapy interventions centered on enhancing mobility, strength, balance, and overall functional ability

Collaborating seamlessly with nurses, physicians, case managers, and fellow rehab professionals to ensure optimal patient outcomes

Educating patients and their families about therapeutic objectives, discharge strategies, and exercises for home

Meticulously documenting evaluations, treatments, and advancements while adhering to facility and regulatory standards

Actively participating in care team huddles and interdisciplinary meetings to promote coordinated and comprehensive care

To excel in this role, you'll need:

A Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

State licensure as a Physical Therapist, with CPR certification being a strong preference

Specialized certifications such as neurological or geriatric being an advantage, but not mandatory

A solid foundation of experience is strongly preferred

The capability to address the needs of patients facing complex medical and mobility challenges

Outstanding communication skills and the spirit of teamwork

Comfort with digital charting and clinical documentation systems
