Description

Conduct thorough assessments of patients in either inpatient or outpatient environments and formulate individualized treatment plans

Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function

Work collaboratively with nursing staff, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes

Guide patients and families through therapeutic objectives, discharge processes, and home exercise regimens

Engage in interdisciplinary team meetings and care rounds to ensure seamless patient care

Education

Doctorate in Physical Therapy (DPT) or a similar accredited degree

Certifications

Current state licensure as a Physical Therapist

CPR certification preferred

Additional specialized certifications (neurological, geriatric) appreciated but not required

Skills

Prior experience is highly desirable
