Description
Conduct comprehensive evaluations for patients in inpatient and outpatient settings, tailoring treatment plans to individual needs
Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functionality
Collaborate closely with nurses, physicians, case managers, and other rehabilitation team members to ensure optimal patient outcomes
Provide education to patients and their families regarding therapeutic objectives, discharge planning, and at-home exercises
Maintain accurate documentation of evaluations, treatments, and patient progress in accordance with facility and regulatory standards
Engage in interdisciplinary discussions and team rounds to ensure cohesive and coordinated care
Education
Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution
Certifications
Valid state licensure as a Physical Therapist
CPR certification highly valued
Specialty certifications (such as neurological or geriatric) are a plus but not essential
Skills
Prior experience in physical therapy is strongly preferred
Ability to manage complex medical and mobility challenges with confidence
Excellent communication abilities and a strong spirit of teamwork
Proficiency in digital charting and clinical documentation technologies
