Description
Conduct comprehensive evaluations for patients in both inpatient and outpatient environments and create personalized treatment plans accordingly.
Implement physical therapy interventions focused on enhancing mobility, strength, balance, and overall function.
Work collaboratively with nursing staff, physicians, case managers, and other rehabilitation experts to maximize patient outcomes.
Educate patients and their families about therapeutic objectives, effective discharge plans, and appropriate home exercises.
Maintain thorough documentation of evaluations, treatments, and patient progress aligned with facility and regulatory standards.
Engage in care team rounds and interdisciplinary meetings to ensure that all patients receive coordinated care.
Education
Doctorate in Physical Therapy (DPT) or its equivalent from an accredited program.
Certifications
State licensure as a Physical Therapist.
CPR certification is preferred but not mandatory.
Additional specialized certifications (such as neurological or geriatric) are advantageous but not required.
Skills
Previous relevant experience is strongly preferred.
Skilled in treating patients with complex medical and mobility challenges.
Excellent communication and teamwork abilities.
Adept at using digital charting and clinical documentation systems.
