Description
Assess patients in both inpatient and outpatient modalities and develop tailored rehabilitation plans
Implement interventions that enhance mobility, strength, balance, and overall functional capability
Work in tandem with physicians, nurses, case managers, and rehabilitation specialists to ensure optimal patient outcomes
Guide patients and families through educational sessions regarding treatment objectives, discharge criteria, and at-home practices
Maintain accurate documentation of evaluations, treatments, and progress as per healthcare regulations and standards
Engage actively in team meetings and interdisciplinary discussions for seamless patient care coordination
Education
Doctorate in Physical Therapy (DPT) or equivalent qualification from an accredited institution
Certifications
Current state licensure as a Physical Therapist
CPR certification is preferred
Additional certifications in specialized areas (neurological, geriatric) are advantageous but not mandatory
Skills
Previous clinical experience is highly desirable
Capability to manage patients with intricate medical and mobility challenges
Excellent collaborative and communication abilities
Comfortable with digital platforms and electronic health documentation systems
