Job description
Enhance Rehabilitation in Wisconsin seeks a qualified Physical Therapist to join our team in a SNF setting. You will evaluate and treat patients following an individualized plan of care while providing excellent patient and family communication.
You will collaborate with facility staff, supervise Physical Therapist Assistants, and complete timely documentation including evaluations, re-evaluations, discharges and daily progress notes to ensure high-quality outcomes.
