Job description
New Vista Of The Bluegrass Inc in Louisville, KY is seeking a Social Service Child Case Manager to support the IDD First Steps program in a hybrid full-time role. The KEIS Service Coordinator will coordinate and monitor early intervention services, conduct intake, referrals, and family assessments, and advocate for families at IFSP meetings.
Travel across seven counties, maintain timely case notes in the TOTS system, meet billing productivity goals, and connect families with community resources.
