Job description
Partners in Care Foundation supports Kaiser Permanente Emergency Department patients and dually eligible Medi-Cal/Medicare members as Care Navigators. The role focuses on real-time engagement, care coordination, and linkage to services within the ED and post-discharge.
The Care Navigator works with ED leadership, social work, CCM, TCS, and community partners to improve care coordination, reduce avoidable ED utilization, and connect members to ongoing supports and resources.
