Job description
Christian Hospital in St. Louis, MO is seeking a compassionate RN Case Manager to coordinate patient care, facilitate safe discharges, and connect patients with post-acute resources. This full-time role in days requires collaboration with ED teams to reduce avoidable admissions. The Case Management Department provides comprehensive services.
RN licensure is required; BSN preferred. EPIC experience is a plus. Weekend and holiday rotation applies.
