Emergency Department Care Navigator – Duals & Medi-Cal

Job description

The Care Navigator will be embedded within the Sacramento Medical Center Emergency Department to support Kaiser Permanente Medi-Cal members and dually eligible Medicare/Medi-Cal members.

The role focuses on real time member engagement, care coordination, and linkage to services, working with ED leadership, care coordination teams, social work, Transitional Care Services, Complex Case Management, Medicare teams, and community partners to improve outcomes.
