Job description
Advocate Health is seeking a pharmacist to oversee pharmaceutical care, design and monitor medication regimens, and provide patient counseling to patients and caregivers. The role includes ensuring safety, accuracy of orders, and involvement in emergencies while supporting formulary adherence and QA activities.
The ideal candidate holds a PharmD (or NABP FPGEC) with state license and relevant certifications, and brings strong communication and organizational skills for effective collaboration
