Job description
Martin Luther King, Jr. Community Hospital in Los Angeles seeks an RN Care Manager for our Emergency Room to coordinate patient care from arrival through safe discharge.
You will assess patients, develop and update care plans, and collaborate with physicians, social workers, and the interdisciplinary team to ensure timely services and smooth transitions of care. You will educate patients and families, advocate for health equity, and connect individuals with community resources.
