Active, unrestricted MI license
Clean DEA/Malpractice/NPDB (no current or unresolved cases or large payouts in past 7 years)
BLS certification
EMR: Meditech Travel & Housing provided Malpractice Insurance provided Rate: Not set; Please provide me your going rate
