The Opportunity
California emergency medicine usually arrives as a package deal: a department that never slows down, and a cost of living that eats the premium you earned for tolerating it. Victor Valley is the other version. About two patients an hour, single physician coverage, 12-hour shifts, in a High Desert market where a physician's income still buys a house with land around it. The hospital has been here since 1967 and serves communities with nowhere else to go which is exactly why the work matters and why the pace stays survivable.
What You'll Own
The whole department. Single physician coverage at roughly two patients an hour.
Your own codes and rapid responses. There is no separate team when it's called, you go.
A broad adult case mix. Pediatrics stays under 10% of volume, and OB is on site rather than something you manage alone.
Stabilize-and-move decisions. No neurosurgery and no cardiothoracic surgery here, so STEMI and higher-acuity trauma transfer out. You'll make that call often and without being second-guessed afterward.

Active California license
Willingness to run codes and RRTs as the only physician in the department
Nice to Have
ABEM or AOBEM certification
Rural, critical-access or High Desert experience
Comfort building transfer relationships you'll rely on weekly

$435,000 $470,000 annually at full-time volume (1,560 hours), independent contractor, inclusive of the RVU component.

You're the only physician on, you run your own codes, and your sickest patients leave by helicopter rather than going upstairs. That combination is either exactly the autonomy you've been looking for or precisely the thing you'd hate. Better to decide that now than in month three. One practical warning: California does not participate in the Interstate Medical Licensure Compact, so if you are licensing in from another state, start that process now it is the long pole in every California hire we make, and it is the single most common reason a good candidate and a good job miss each other.
