Job description
CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to support patients in their homes in Elizabethtown, KY. You will implement the treatment plan created by the Physical Therapist and deliver non-complex manual therapy and modalities to improve function in geriatric patients.
You’ll document progress weekly, participate in staff education, and coordinate with the PT and Branch Director on equipment and supply needs.
