Job description
CenterWell Home Health is seeking a licensed Physical Therapist to provide in-home rehabilitation, assess patient needs, and document progress. You will collaborate with physicians, clinicians, PTAs and aides to implement personalized therapy plans aimed at restoring function and independence.
The role is patient-facing within the Humana CenterWell Home Health network, with a focus on safety and high-quality care. A current PT license and CPR certification are required.
