Job description
CenterWell Home Health is seeking a licensed Physical Therapist to deliver in-home care, helping patients regain strength, mobility and independence in their own homes. You will assess, plan and administer prescribed therapy focused on functional improvement and patient goals.
Reporting to the Clinical Manager, you will collaborate with a multidisciplinary care team, document progress, and communicate with physicians, patients and families to ensure optimal outcomes.
