Job description
Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to evaluate and treat patients in the home setting. You will use physical modalities to improve mobility and functional independence, and work under physician orders with the branch director’s supervision.
Responsibilities include implementing plans of care, documenting progress, and coordinating with families and care teams to maximize patient outcomes in a home care environment.
