Job description
CenterWell Home Health is seeking a licensed Physical Therapist to provide in-home care and help patients regain strength and independence. You will report to the Clinical Manager and plan, implement, and document PT treatments across patient homes.
You will assess function, develop rehab programs, supervise PTAs and aides, and communicate progress to families and the care team. This is a 40-hour-per-week, full-time role with a pay-per-visit component within the posted range.
