Job description
CenterWell Home Health seeks a licensed Physical Therapy Assistant to administer and support home-based physical therapy under the supervision of a Physical Therapist. You will deliver hands-on treatment in patients’ homes, document progress, and coordinate with the care team to promote independence and functional gains.
The role requires an active PTA license, valid driver’s license, CPR certification, and strong teamwork. Fluency in English and geriatric care knowledge are essential.
