Job description
Ohio Living in Canfield, OH, seeks an experienced RN Case Manager for Home Health to plan, coordinate, and deliver quality care in patients' homes while ensuring compliance with regulatory standards. You will assess patient needs, develop care plans, and oversee their execution with the care team.
