Job description
CenterWell Home Health, a Humana company, is seeking a Home Health Speech Language Pathologist to provide expert in-home SLP services under the Clinical Manager. You will evaluate patients, develop and adjust plans of care, and support rehabilitation to improve communication and swallowing abilities.
You will administer assessments, document changes, and collaborate with families and healthcare staff to ensure high-quality, confidential care.
