Job description
The Home for Little Wanderers seeks a qualified Family Partner to support families and caregivers of children with special needs, delivering guidance and coaching to implement care plans in home and community settings.
You will collaborate with the Care Planning Team, perform weekly home visits, and coordinate with treatment providers while facilitating access to community resources. A valid driver’s license and access to a vehicle are required, and extensive training is provided.
