Job description
St. Croix Hospice is seeking an experienced Registered Nurse Case Manager to lead the patient’s hospice care team and develop the Plan of Care in collaboration with the Hospice Medical Director, patient, and family.
You will provide day-to-day patient care, manage treatments, and coordinate care with physicians and the care team in home settings to enhance comfort and quality of life. The role requires current RN license, ability to pass background checks, and a valid driver's license.
