This is a rewarding opportunity to make a meaningful impact in the lives of patients while growing professionally within a supportive, mission-driven environment.

Responsibilities

Provide on-site endocrinology consults in nursing home settings, including initial assessments for new admissions and complex endocrine conditions such as diabetes, thyroid disorders, osteoporosis, and adrenal disorders, as well as routine weekly follow-up visits and care adjustments based on findings.

Conduct comprehensive evaluations by gathering medical histories, performing endocrine-focused physical exams, and ordering and interpreting labs such as A1C, thyroid panels, metabolic panels, and bone density tests.

Collaborate with endocrinologists, interdisciplinary teams, nursing staff, and facility providers to develop treatment plans, manage complex metabolic conditions, and reduce hospital readmissions.

Maintain familiarity with CMS regulations and NCQA HEDIS guidelines.

Participate in quality improvement programs aimed at improving chronic disease management in skilled nursing facilities.

Active and unrestricted NP license.

DEA license required.

CPR certification (BCLS or BLS).

Master’s degree in Nursing (NP applicants).

Graduation from an accredited NP program.

Preferred familiarity with EMR systems such as Sigma, Visual, or PCC (training provided).

Strong organizational, communication, and time‑management skills.
