Manage a patient panel with 85% diabetes and thyroid care (new diagnoses + longitudinal management)
Collaborate with a dedicated diabetes care team , including pharmacy support
Conduct comprehensive consultations, develop individualized care plans, and guide patients through lifestyle changes
Work closely with referring providers and specialists in a truly integrated system
Enjoy predictable, weekday hours (Mon Fri, 8:00 AM 4:30 PM) no nights, no weekends
