SCHEDULE
Coverage Type: Inpatient and Outpatient
Schedule Type: Clinic and Call
Call Type: 24-Hour Beeper Call
Dates of Need: 1 Week per month
Shift Times: 8a-5p
Lunch TBD
Provider Min # Shifts Required: 7-14
Guaranteed Hours per Day: 8
Hours Worked for OT to apply: 8 hours per day
Rates to be Filled Out on Bid: Clinic and Call: Regular, Orientation, Overtime, Holiday, Night Call, 24 Hour Call, Call-Back.
CREDENTIALING
Current CV (must be uploaded to bid)
License: Active or IMLC
DEA Required: Yes
Boards: BC only

Other: Please include references with presentation. This group is wanting to ensure the providers who are presented are solid candidates prior to dedicating resources to obtaining hospital privs. We will not send presentations to the site without references included.
ADDITIONAL DETAILS
Needing coverage from 1 provider one week a month for ongoing, consistent care. Standard outpatient clinic schedule, 8-5 M-F. Providers will have 1 hour for new patient and Diabetic appointments and 30 minute follow up appointments. 8-10 patients per day. Minimal rounding on weekends during call coverage.
