Oak Street Health is seeking motivated New Graduate Nurse Practitioners for a immersive Fellowship within our Healthcare & Medical Services department. Fellows deliver team-based primary care to Medicare patients in underserved communities while receiving structured mentorship, didactic training, and supervised clinical practice. Responsibilities include assessments, chronic disease management, preventive care, and patient education using an EHR-driven, population health model.
