Job description

Evergreen - PT Nurse Practitioner Sonoma

PINE PARK HEALTH MANAGEMENT INC Sonoma, California, United States

About this position

Pine Park Health is a value-based primary care practice revolutionizing healthcare for senior living community residents. We empower seniors to get healthy, stay healthy, and lead lives they love through a care model designed with everyone in mind — patients, families, community staff, providers, and payers.

Our approach brings healthcare directly to seniors' homes, offering prevention, screening, chronic condition management, lab work, and diagnostic testing in the comfort of their apartments. With weekly community visits, our teams provide consistent care while collaborating closely with facility staff to address comprehensive health needs. We've eliminated unnecessary barriers to urgent care through same/next-day appointments, helping seniors avoid emergency rooms and hospitalizations where they risk exposure to additional health concerns.

Today, over 185+ communities across Arizona, California, and Nevada trust Pine Park Health, and we're rapidly expanding our reach and impact.

If you're mission-driven and passionate about transforming senior healthcare, this is your opportunity to make a meaningful difference!

The Opportunity

Are you a Nurse Practitioner passionate about geriatric care and looking for a role that honors the complexity of your work while respecting your life outside of medicine?

Pine Park Health is actively building our pipeline of exceptional part-time Nurse Practitioners to deliver care across our SF Bay Area market. While we may not have an opening available right now, we're growing rapidly, and we want to connect with clinicians like you who are interested in joining us when the timing aligns.

If you're thrilled about working with smaller, complex patient panels, contributing to a value-based care model, and collaborating with a dynamic multidisciplinary team, we'd love to hear from you — even if you're not ready to move immediately.

Our care model:

This person-centered approach leverages care coordinators and dedicated RNs who handle triage and administrative work, so you focus your clinical expertise where it matters most. You'll practice in comfortable residential settings, develop meaningful patient relationships, and have a real voice in shaping clinical protocols and innovations in our value-based model

What You'll Do

Provide onsite primary care services to older adults living in Senior Living Communities

Develop and execute detailed person-centered care plans for all primary care patients

Work with our Medical Director to identify and develop policies addressing clinical issues, emerging diseases, and practice culture

Provide monthly Chronic Care Management (CCM) visits and documentation

Identify ways to collaborate with senior living management to improve patient outcomes and care delivery

Contribute to our clinical innovation and quality improvement initiatives

What We're Looking For

Nurse Practitioner certification with passion for geriatric care (prior geriatric experience preferred)

Adaptability and resilience in a dynamic healthcare startup environment

Willingness to provide onsite clinical services across multiple senior living communities

Dedication to complex care patients and our holistic care model

Collaborative mindset with appreciation for multidisciplinary teamwork

Exceptional communication skills for interacting with patients, families, care teams, and community partners

Tech-savvy approach to embrace digital care tools (EMR, remote monitoring, care coordination)

Flexibility for part-time scheduling and local travel between communities

Why You'll Love Us

Mission-driven team — Smart, heart-centered, low-ego group united by our commitment to transforming senior healthcare

Meaningful work — You tackle complex challenges that directly impact seniors' lives

Ownership mindset — Empowered to innovate, supported by continuous learning and constructive feedback

True flexibility — Part-time scheduling that respects your life outside of work, with a hybrid model balancing in-person and remote work

Collaborative culture — Robust clinical support from dedicated RNs and Care Coordinators

Community — Regular social events and offsites to celebrate wins and build relationships

Here’s What Comes With It

Base salary range: $135,000–$160,000 (plus generous quarterly bonuses, equity, and benefits)

Generous PTO: 10 holidays + 15 vacation days + paid sick leave + 8 weeks paid parental leave

Professional development: Dedicated funding to advance your clinical expertise

Wellness support: Monthly wellness allowance + 401(k) + Flexible Spending Accounts

Practical perks: Mileage reimbursement, company-issued iPhone, travel support

Final compensation will reflect education, experience, certifications, geographic location, and market data
