Job description
The Chatham Hills Subacute Care Center is actively seeking an Experienced Physical Therapist to lead rehabilitation services at the facility. The PT will assess referred patients, design and supervise treatment programs, and document progress in collaboration with the physician.
Strong documentation, team coordination, and patient-centered care are essential. Candidates should have an accredited PT degree and a valid state license, with preference for long-term/geriatric care experience.
