Job description
Chatham Hills Subacute Care Center is seeking an experienced Physical Therapist to join our facility. The PT will assess referred patients, establish treatment programs, and document patient progress under physician direction.
The role requires graduation from an accredited PT program with a valid license and the ability to supervise Physical Therapy Assistants. Experience with long-term/geriatric care and strong documentation skills are preferred.
