Job description
The Chatham Hills Subacute Care Center in New Jersey seeks an experienced Physical Therapist to assess referred patients and provide direct PT care within a multidisciplinary team. You will establish treatment plans with clear goals, document patient progress, and educate patients and families on therapy options.
The role requires an accredited PT program graduate with a valid license, experience in geriatric or long-term care, and strong documentation skills.
