Job description
Chatham Hills Subacute Care Center is seeking an Experienced Physical Therapist to assess and treat patients in a subacute care setting. You will lead therapy plans, provide direct patient care, and educate families on rehabilitation methods.
The role includes supervising PTAs, collaborating with a multi-disciplinary team, and ensuring compliant documentation and reporting. A valid NJ license and LT care experience are preferred.
