Job description
Chatham Hills Subacute Care Center seeks an experienced Physical Therapist to assess patients requiring rehabilitation and provide direct patient care. You will establish treatment plans under physician direction and set achievable goals while documenting patient progress and educating families.
Required are a PT license, graduation from an accredited program, and experience supervising assistants. Familiarity with long-term/geriatric care and HIPAA compliance is valued in a collaborative care
