Job description
Chatham Hills Subacute Care Center in New Jersey seeks an experienced Physical Therapist for our facility to provide comprehensive rehabilitation services.
You will assess patients, develop treatment plans, document progress, and educate families while supervising Physical Therapy Assistants and collaborating with the care team.
Experience with PPS scheduling and Case Mix Index is preferred; strong documentation and patient-centered care are essential.
