Job description
Southeast Primary Care Partners seeks a Physician Assistant or Nurse Practitioner to join a collaborative primary care team in Georgia. The APP will manage chronic and acute conditions, work with physicians and RN case managers, and support outpatient and inpatient needs as part of a coordinated care model.
The ideal candidate will have Georgia PA/NP licensure, prescriptive authority, BLS, and at least two years in primary care, contributing to a patient-centered, team-based approach.
