Job description
Chatham Hills Subacute Care Center is seeking an Experienced Physical Therapist to provide comprehensive rehabilitation services. The PT will assess referred patients, establish treatment plans, and educate families while coordinating with a multi-disciplinary team.
The role requires valid licensure, supervision of PT Assistants, and strong documentation. Prior long-term or geriatric care exposure is preferred. Confidentiality and patient-focused care are essential.
