Job description
Molina Healthcare is seeking an experienced Care Manager to support Medicaid members in Cook County, with responsibilities in nursing facilities, care planning, and coordination alongside nursing staff onsite in Downtown Chicago.
Ideal candidates have at least 2 years of healthcare experience, a related bachelor’s or master’s degree, and proficiency with Microsoft Office and Office 365. Travel up to 25-40% may be required, and the role is Monday–Friday, 8 AM–5 PM CST.
