Job description
Hartford Healthcare seeks a Fairfield County Revisit Nurse to provide compassionate home-based follow-up care to patients after hospital or clinic discharge. You will complete skilled nursing assessments, deliver treatments and medication management, and closely monitor patient progress to reduce readmissions. Collaborate with an interdisciplinary team, update care plans, and educate patients and families.
