Job description
Hackensack Meridian Health seeks a Palliative Nurse Navigator to coordinate and provide high-quality palliative care in collaboration with clinicians. The role includes assessing patient and family needs, facilitating access to services, and ensuring continuity of care across settings.
The candidate will have an RN license in New Jersey with at least 5 years of clinical experience, strong communication skills, and proficiency in EPIC and Microsoft Office.
