Job description
Health Connect America in Ohio seeks qualified clinicians to provide Family Centered Treatment (FCT) services in home and community settings, partnering with families to achieve treatment goals. You’ll maintain clinical records, coordinate services, and participate in team-based planning while upholding fidelity to the FCT model.
Applicants should have a Bachelor's in human services or related field, 2 years’ experience with youth or families, and preferably an active FCT certification; Ohio
