Job description

Overview

Some careers are just jobs. This one is different. At Georgia HOPE , the work you do every day creates meaningful change for children, families, and individuals who need it most.

We're a Georgia-based behavioral health provider on a mission to instill hope, empower lives, and strengthen communities. Our team specializes in supporting individuals facing emotional and behavioral challenges—often those who have experienced trauma, instability, or significant life stressors. We deliver services directly in the homes, schools, and communities where our clients live and grow, because real progress happens when care is personalized, strengths-focused, and meets people where they are.

As part of the Health Connect America family—a COA-accredited organization serving children, families, and adults across nine states—you'll have the resources, support, and professional development of a larger organization, paired with the close-knit, mission-focused culture that defines our work.

If you’re looking for a career where your impact is visible, your growth is supported, and your work truly matters, you’ll find it here.

Health Connect America Family of Brands

Responsibilities

If you're someone who believes that real change happens in real life — not just in an office — this role was built for you.

As a Family Centered Treatment (FCT) Specialist at Georgia HOPE, you'll deliver meaningful, evidence-based services where it matters most: in the homes and communities of the children and families you serve.

What You'll Do

As an FCT Specialist, you'll work with a caseload of children, adolescents, and families, guiding them through a structured, research-backed treatment model designed to create lasting change. Your work will be hands-on, relationship-driven, and grounded in best practices. Day to day, you can expect to:

Deliver Family Centered Treatment services primarily in home and community settings, meeting families where they are — literally and figuratively

Guide children and families through the four phases of the FCT model, helping them set goals, build skills, and move toward sustainable well-being

Serve as a central point of coordination for each family's Treatment Team, collaborating with medical, behavioral health, school, and community partners to ensure seamless, whole-person care

Develop, implement, and continuously refine individualized Service Plans in partnership with the families you serve

Connect families to community resources, referrals, and aftercare supports that extend the impact of treatment beyond your time together

Maintain timely, accurate documentation in the Electronic Health Record, keeping records that reflect the quality of care you provide

Participate in Quality Assurance activities that help make our programs stronger for every family we serve

What to Expect

This is community-based work, which means a flexible schedule that accommodates families' lives. Like most roles serving children and families in the community, this position includes 24/7 on-call availability for crisis support — because sometimes families need a steady, trusted voice outside of regular hours. In practice, crises don't define the day-to-day experience but knowing you're there when it counts is a meaningful part of the impact you make in this role.

Your Path to FCT Certification

FCT Certification is required for this role and must be completed within one year of hire. Georgia Hope supports you every step of the way — through structured checkoffs, e-learning trainings, and a team that's invested in your professional growth. Becoming FCT-certified isn't just a requirement; it's a credential that sets you apart in the field.
