Job description
Health Connect America, a COA-accredited provider, seeks clinicians to deliver Family Centered Treatment in homes and communities. You will coordinate treatment with families, complete service plans, and ensure documentation in EHR with fidelity to the FCT model.
Preferred candidates hold a related Bachelor’s degree and FCT certification, with experience serving youth and families across multiple state requirements.
