Job description
Health Connect America is seeking a Family Centered Treatment Specialist to deliver FCT services to children, adolescents and families, primarily in home and community settings with a strong family focus. The role requires FCT certification within one year, a Bachelor's degree in human services or related field, and two years’ experience.
You will develop and monitor Service Plans, document care, and collaborate with Treatment Team members, including on-call coverage.
