Job description
Institute for Community Living Inc. in Brooklyn, NY seeks a Mental Health Comprehensive Medicaid Case Manager for the ACT Team to support adults with serious mental illness in community settings.
Under supervision, you provide counseling, case management, housing and employment support, crisis response, and family psychoeducation, working in a mobile multi-disciplinary team. Applicants must have a Master’s in Social Work; NYS licensure is preferred.
