Key responsibilities include:

Education and certification requirements include:
Doctor of Medicine (MD) or Doctor of Osteopathy (DO) from an accredited institution.
Completion of a recognized residency in Family Medicine.
Licensure and certifications include:
Board Certified in Family Medicine.
Possess (or be eligible for) an Illinois medical license.
Preferred certifications include BLS and ACLS.
