Position Highlights:
Physician-led, team-based care model
Flexible schedule (4 or 4.5 days/week available)
Focus on quality and outcomes, not volume
Strong support staff and care coordination
Advanced technology with national resources
Leadership and teaching opportunities
Provide comprehensive primary care to a diverse patient population
Collaborate with interdisciplinary care teams
Perform in-office procedures (optional)
Participate in quality and population health initiatives
Educate and engage patients in their care

Required Qualifications:
Unrestricted medical licensure in the state of Indiana or able to obtain prior to start
Board certified or board eligible in Family Medicine or Internal Medicine or Med-Peds
Active and unrestricted DEA License or ability to obtain prior to start
